Delaware Juvenile Custody Visitation Forms Can Now Be Done Online

Delaware Juvenile Court makes forms available for domestic relations cases. 

These custody, support and visitation cases take place when the parties are not married. If the parties are married the parties must go to Common Pleas Court Domestic Relations to deal with custody, support and visitation issues.

All forms are in Adobe PDF format. You will need Acrobat Reader on your PC to be able to view and print these files. To access the forms...

Go to https://clerkofcourts.co.delaware.oh.us/clerk-forms/

  1. Find the "Domestic Relations" Section
  2. Choose the form or forms you need. (example: MOTION FOR CHANGE OF PARENTING TIME (COMPANIONSHIP AND VISITATION) AND MEMORANDUM IN SUPPORT https://clerkofcourts.co.delaware.oh.us/wp-content/uploads/sites/9/2018/04/Form23.pdf)
  3. Fill in the highlighted areas on the forms to the best of your ability. You may or may not need to complete all pages.
  4. Print the forms and sign the docs where indicated.

Print on 8.5’ x 11’ white paper, signal sided, black ink only. Now you're ready to turn them into your attorney for final review and submission to the court.
